A motion for reconsideration shall be filed pursuant to a petition by the petitioner
or a representative thereof with the court rendering the judgment subject of
reconsideration in accordance with applicable case-filing procedures, provided
that the petition includes particulars of the judgement subject of
reconsideration, a summary thereof, and grounds for filing said motion.
